HC Melbourne Post Season Survey 2022

As a valued member of the HC Melbourne team we would like to seek feedback about your European Tour and/or Commonwealth Games experience(s). 

The aim is to use the feedback to improve our High Performance Program and Olympic preparation phase for Paris 2024. 

The survey will take around 15 minutes, with each question requiring you to rate your response on a 3 point scale: 

😀 - exceeded expectations
😐 - effective, as expected, fit for purpose
🙁 - ineffective, below required level
NA - Not Applicable

You will also be asked to provide input on where there are improvements to be considered as well as suggested solutions or alternatives - This is optional but will help in making improvements going forward. 

Please read the headings and questions carefully as you will find that the same question is often asked twice but one is referring to your reflections from the European Tour and the other to your reflections from the Commonwealth Games. 

Your responses will be collated and remain de-identified and confidential in any reporting. Themes identified from the survey will be discussed in facilitated workshops. Once all feedback has been received and analysed, Hockey Australia will provide a summary of findings to all athletes, coaches and performance support team members.

   1. Preparation, Performance Health and Your Performance 
   2. Team Culture, Dynamic & Interactions 
   3. Administration & Logistics
