Participant Waiver & Terms of Conditions

By completing this registration, I (as the parent or legal guardian of the registered participant) agree to the following terms:

• Parental Supervision: I understand that parent/guardian supervision is mandatory and agree to remain on-site at Seymour College P-12 for the entire duration of every session.

• Risk & Liability: I acknowledge that participation in physical sports involves inherent risks. To the extent permitted by law, I release Hockey Victoria, Seymour College P-12, SeymourToyota, and their staff or volunteers from liability for any personal injury, loss, or property damage occurring during the program.

• Medical Authorization: In the event of an injury or emergency, I authorise Hockey Victoria staff to obtain necessary medical care or emergency transport (including ambulance services) if I cannot be contacted immediately, and agree to cover any associated costs.

• Media & Promotional Consent: I grant permission for photos and videos of my child taken during the program to be used by Hockey Victoria and its Community Partners (Seymour Toyota) for social media, marketing, and promotional purposes.

• Program Minimums: I understand that a minimum of 12 participants is required for the program to proceed and that sessions may be modified or cancelled due to numbers or severe weather.
