Junior Cobras Program – Terms & Conditions and Event Waiver

By completing this registration, you (as the parent or legal guardian of the participating child) acknowledge and agree to the following terms and conditions:

1. Supervision & Parental Responsibility

• A parent or legal guardian must remain on-site for the entire duration of each session. Hockey Victoria staff and coaches are responsible for session facilitation only and do not provide OSHC or childcare supervision.

2. Risk Assumption & Participant Safety

• You understand that participation in field hockey and sports activities involves inherent physical movement, exercise, and minor risks of physical injury.

• You confirm that your child is physically fit and capable of participating in entry-level sports activities. You agree to disclose any relevant medical conditions, allergies, or special needs during registration to assist our coaching team.

3. Medical Emergency Authorisation

• In the event of an injury or medical emergency involving your child, you authorise Hockey Victoria staff and qualified coaches to administer basic first aid as necessary.

• In the unlikely event of an injury or medical emergency where a parent or guardian cannot be immediately located on-site, you authorise Hockey Victoria staff to administer basic first aid and seek emergency medical treatment or transport at your expense.

4. Media & Photography Consent

• You acknowledge that photographs and video recordings may be taken during the program by authorised Hockey Victoria staff for promotional, marketing, and social media purposes.

• Note: If you do not wish for your child to be photographed or filmed, please notify the lead coach on-site prior to the start of the session.

5. Cancellations & Program Changes

• Session delivery is subject to minimum participant registration requirements (12 participants) and safe weather conditions. Hockey Victoria reserves the right to cancel or reschedule sessions if necessary, and will communicate changes via the email address provided at registration.
