HCM Heroes Holiday Program: Participant Waiver & Terms of Induction

1. Physical Risk & Medical Authorisation

I understand that hockey is a contact sport involving inherent risks of injury. I represent that the participant is physically fit to participate in the Junior Cobras Holiday Program.

• Emergency Treatment: In the event of an injury or illness where I cannot be contacted, I authorise Hockey Victoria (HV) and Melbourne Cobras staff to seek medical, hospital, or ambulance treatment for the participant. I agree to meet all costs associated with such treatment and transportation.
• Medical Conditions: I have fully disclosed all allergies, medical conditions (e.g., Asthma, Anaphylaxis), and dietary requirements on the registration form and will provide any necessary Action Plans upon arrival.

2. Media, Content & "The Press Gallery" Consent

I understand that a core component of the Melbourne Cobras experience involves media roleplay, social media "interviews", and professional photography.

• Content Rights: I grant HV and Melbourne Cobras the irrevocable right to use the participant’s name, image, and voice recorded during the program for promotional, marketing, and social media purposes across all platforms (including but not limited to Instagram, TikTok, and the official website).
• Post-Match Station: I consent to the participant’s "Post-Match Interview" being shared on official club digital channels.

3. Data Usage & Privacy

I consent to the participant’s (and my) details being added to the Hockey Victoria and Melbourne Cobras databases for the purposes of program fulfilment, match-day communications, and future development opportunities. Data will be handled in accordance with the [Hockey Victoria Privacy Policy].

4. Equipment & Facility Conduct

• Safety Gear: I acknowledge that the participant must wear a mouthguard and shin guards to participate in any on-pitch activities.
• The "Contract": I understand that participants are expected to follow the "Melbourne Cobras Code of Conduct". HV/Cobras reserves the right to remove a participant from the program (without a refund) if their behaviour is deemed unsafe or consistently disruptive to the "Inner Sanctum" experience.

5. Release of Liability

To the extent permitted by law, I release and hold harmless Hockey Victoria, Melbourne Cobras, and Casey Cannons Hockey Club, including their staff and volunteers, from any and all claims, demands, or causes of action resulting from the participant’s attendance, except in cases of gross negligence.
